PURPOSE: Performs under the supervision of the Registered Nurse, Nurse Manager, Administrative or Relief Supervisor. Responsible for other duties that may arise while on tour of duty. Will act in a supportive role to nurses and/or physicians. JOB RELATIONS: A.        Responsible to the Nurse Manager of designated unit. Ultimately responsible to the Nurse Manager and Director of Nursing. B          Supervises no other employees; may act as preceptor to new Certified Nursing Assistants hired. C.        Inter-relationships with Ancillary Departments is essential.

Duties
ESSENTIAL JOB FUNCTIONS: 1.         Assists with personal care needs of patients. 1: 1      Assists the nursing staff, when requested, in performing the following  patient hygiene needs: a.         Changing bed linens (unoccupied bed) b.         Holding patients for occupied bed changes c.         AM/PM Care. 1:2       Maintains patient safety by elevating side rails, placing call light within reach, and applying restraining devices when indicated according to established procedures with no reported errors. 1:3       Provides sufficient protection and physical support to patients while in bed or chair, on a commode, ambulating, transferring, or being transported as evidenced by no reported errors. 1:4       Demonstrates consistent ability to provide for patient positioning or ambulatory needs according to procedure and direction. 2.         Assists in providing for nutrition and elimination needs of patients. 2:1       Ensures trays are checked against patient identification and diet according to procedure with no reported errors. 2:2       Provides for restricted, supplemental fluids/feedings based on patient plan of care. 2:3       Correctly calculates intake and output to include: a.         P.O. liquids b.         \% of diet eaten c.         Voids d.         Foley Catheters e.         BM’s 2:4       To assist the nursing staff with the following: a.         Removing a patient from a bedpan b.         Assisting to ambulate a patient to bedside commode c.         Assist with the insertion of a Foley catheter. 3.         Maintains a safe unit environment. 3:1       Adheres to nursing safety plan (i.e., body mechanics, universal precaution, fall preventions) with no error trending. 3:2       Maintains patient safety from hospital-acquired infections by adhering to infection control policies and procedures. This includes isolation. Uses  universal precautions. 3:3       Assists in providing for a clean and stocked patient environment as requested by nursing staff to include: a.         water - ice b.         linen c.         patient care supplies. 3:4       Obtains and returns/disposes of equipment according to direction and  requested/established time frames. 3:5       Ensures that supplies and equipment returned to central sterile are free of excessive soiling as indicated by no more than three complaints per month. 3:6       Answers patient call system promptly within 3 rings. 3:7       Enforces the traffic control/visitation policies. Notifies nurse of any problems. 3:8       Assures that routine equipment is functioning prior to and during use as  demonstrated by equipment failure reports and notifying charge nurse. 3:9       Maintains orderliness of supplies, paperwork, and equipment by using appropriate storage areas at all times. 3:10     Cleans charts and keeps an organized, clean desk area. 4.         Assists the nursing staff in performing specific procedures according to plan of care under the direction of licensed nursing staff. 4: 1      Demonstrates consistent ability to perform routine patient treatments within scope of practice and according to procedure to include:a.         height b.         weight c.         temperature, pulse, respirations d.         blood pressure 4:2       Documents patient’s TPR, treatments, test results, diet and other unit specific routines correctly on worksheets according to established procedures with no more than 2 errors of recording per month. 4:3       Ensures specimens are properly identified and delivered to correct locations/departments with no more than 3 complaints from receiving  department within one year. 5.         Facilitates admission, discharge and transfer procedures for patients on and off unit. 5: 1      Assists with the preparation of rooms for admission/transfer according to unit guidelines, policies and procedures, and requested time frames with no more than one complaint by patient or staff. 5:2       Assists with the completion of patient admission and transfer to unit by orienting patients to room, bedside unit, and storage for belongings; assorting patient to bed according to established routines as expressed by satisfaction of patient. 5:3       Assists nursing staff, when requested, in correctly transferring patient between bed/chair and stretcher/wheelchair per established time frames as evidenced by no more than two complaints by receiving department. 5:4       Assists with transfer to ancillary department, when requested, as  evidenced by no more than two complaints from receiving department.6.         Contributes to the effective communication and effective operation of the nursing  unit. 6:1       Participates with valid input and suggestions during departmental  meetings. 6:2       Demonstrates the ability to respond to an emergency situation according to  policy and procedure as directed. 7.         Demonstrates a commitment to continued self growth and maintenance of  competency. 7:1       Attends 100\% hospital required inservices. 7:2       Attends 50\% nursing unit meetings. 7:3       Follows established guidelines for vacation and holiday requests with no  more than two deficiencies per year. 7:4       Notifies Director of Nursing, Nurse Manager, or Supervisor when unable  to report to work as scheduled within appropriate time frame. 7:5       Maintains proper appearance by adhering to nursing service dress code. 7:6       Demonstrates ability to use all channels of communication to transmit  information, address concerns, and solve problems. 7:7       Maintains patient privacy and confidentiality. 7:8       Maintains current CPR certification. 7:9       Satisfactorily completes unit assistant skills checklist yearly. 8.         Adheres to and follows the principles of Fishermen’s Hospital’s Quality Customer Relations program. 8: 1      Promotes quality customer relations by identifying both internal and external customers; acknowledging customers promptly; and using expressions that express respect, understanding. and enhance self esteem. 8:2       Creates a positive impression through effective use of telephone skills such as answering the phone by the third ring, identification of self and  department, listening attentively, personalizing the conversation, and  verifying information. 8:3       Creates a supportive climate for customers by allowing customers to  express themselves, addressing their concerns as being real and  encouraging two-way communication. 8:4       Promotes therapeutic interaction with customers by identifying behaviors  resulting from dissatisfaction; then, allowing customers to respond openly,  showing empathy and discussing options. 9.         Where appropriate, performs age specific patient assessment demonstrating  awareness of growth and development for each age group. Demonstrates  knowledge of age specific assessment, recognizing psychosocial levels and developmental patterns. Provides for age appropriate stimulation and learning  needs. 9: 1      Understands importance of family involvement when addressing needs of  pediatric patient. Includes family in planning of patient care. Recognizes  main target of teaching is family, and considers family’s anxiety over sick  child. 9:2       Performs assistant skills specific to pediatric patient. Demonstrates  commitment to obtaining knowledge of all routine and emergency procedures. 9:3       Recognizes and reports suspected cares of child abuse. Demonstrates  awareness of signs and symptoms of child abuse and reports to supervisor and authorities as indicated. 9:4       Performs nursing skills specific to adolescent patient. Demonstrates knowledge of routine and emergency care specific to adolescent patient. 9:5       Applies knowledge of age appropriate needs when assessing/caring for elderly patient. Addresses concerns of adult and elderly patients according  to psychosocial and developmental needs. 9:6       Performs aide skills specific to adult and elderly patient. Demonstrates knowledge of routine and emergency care specific to the adult or elderly patient.
